Isotretinoin and Oral Contraceptive Use in Female Acne Patients Varies by Physician Specialty: Analysis of Data from the National Ambulatory Medical Care Survey.

Abstract Objective: To determine whether OCPs are underutilized in the treatment of acne in women of reproductive age, how use of OCPs compared to use of isotretinoin, and whether adequate use and documentation of OCPs is occurring with isotretinoin. Methods: The National Ambulatory Medical Care Survey (NAMCS) was analysed over the years 1993-2008 for isotretinoin and oral contraceptive use in females aged 12-55 with acne. Results: Isotretinoin was prescribed more often than OCPs at both first visits (4.7% vs. 3.3%) and overall visits (13% vs. 2.6%) for acne. Documentation of OCP or other contraceptive use occurred only 4.1% of the time overall in patients treated with isotretinoin. Specialties varied in both OCP use for acne and contraceptive use with isotretinoin, with ob/gyn specialists most likely to prescribe OCPs and isotretinoin and to report contraceptive use in patients using isotretinoin, and dermatologists least likely to prescribe OCPs. Conclusion: The findings of the current study indicate that OCPs may be underutilized in women with acne. Underreporting of contraceptive use with isotretinoin and variations between specialties in OCP and isotretinoin use indicate a potential for education about the viability of OCPs in acne treatment and the importance of reporting contraceptive use with isotretinoin.

Open Question: How long until I start seeing results?

Firstly, I have some acne, not absolutely horrible, but my skin is definitely not clear! I’ve tried a LOT of products and I already eat a very good, balanced diet, but I still have it. So after talking to nutritionist and other professionals as well as reading a variety of articles, I have decided to stop drinking milk, which is hard as its one of the main things I consume, but kind of necessary. So, from the day I stop drinking milk, how long will it be until I start seeing noticeable results?
Secondly, if I start working out for a minimum of 30 minutes a day with cardio, sit ups, running, yoga, pilates, ect. how long until I see results in both weight loss and muscle build/toning?
Lots of questions, I know, but thanks to anyone who answers!

Abstract Background: Oral isotretinoin is effective for acne vulgaris but concerns remain regarding its adverse effects. Objectives: This study aims to evaluate the safety and efficacy of isotretinoin for acne vulgaris in Asian patients. Methods: We conducted a 4-year retrospective study on all patients with acne vulgaris treated with isotretinoin, between January 2005 to December 2008 at the National Skin Centre in Singapore. Medical records were reviewed for information on patient demographics, acne severity, isotretinoin dose, duration, adverse effects and outcome. Results: There were 2,255 patients, with a mean age of 22.5 years, male: female ratio of 2.5:1 and 82.3% being Chinese. The mean starting dose of isotretinoin was 0.4 mg/kg and on average, patients received 7.8 months of treatment at a mean dose of 0.5 mg/kg. Mean total cumulative dose was 95.6 mg/kg. Vast majority (93.9%) achieved complete remission or substantial improvement. Thirty-eight (2.2%) and 24 patients (2.7%) developed elevated serum levels of alanine and aspartate aminotransferases subsequent to treatment. There were 194 (12.1%) and 80 (4.8%) patients who developed hyperlipidaemia and hypertriglyceridaemia respectively. Isotretinoin was generally well-tolerated, with 6.4% (n=145) discontinuing due to side-effects. Conclusions: This study reaffirms the overall safety and efficacy of oral isotretinoin in Asian patients with acne vulgaris.

Given the multifactorial and complex contributors to acne development, combination therapy is standard of care. By addressing multiple pathogenic factors, combination therapy provides a quicker and more efficacious treatment outcome than monotherapy. Topical retinoids normalize follicular keratinocyte differentiation and are anti-inflammatory. Their use is limited by the potential for cutaneous irritation. Antimicrobials reduce Propionibacterium acnes colonization on the skin and reduce the bacteria’s proinflammatory effects. Topical antibiotics and benzoyl peroxide (BPO) are commonly employed in fixed-dose combination products or two separate medications. BPO has the added benefit of being comedolytic and can minimize the risk for bacterial antibiotic resistance. Like topical retinoids, BPO may cause skin irritation, burning, erythema, and peeling. Managing cutaneous side effects when using multiple products that cause irritation can be a challenge. Careful product selection, dose titration, and patient-directed regimens can help to optimize outcomes. This review presents the latest data on two topical acne products that have demonstrated excellent efficacy and tolerability profiles. In addition, their in vitro profiles suggest the potential for combination use, affording greater dosing flexibility. J Drugs Dermatol. 2012;11(3):313-317.

The scabs I have aren’t the raised kind but they are flat and slowly healing(they are not scars okay i know this for a fact) I drink 2 liters of water everyday already, drink green tea, wash my face 2 times a day and moisturize, and do not wear ANY face make up.so don’t say anything that corralates to any of those.)
P.S. i’m fourteen, girl obviously, and am ssoooo close to clear skin but these scabs are bringing me down….
and pls don’t recommend products… pls keep it somewhat natural sorry for being picky!!!!
